- Work collaboratively with the healthcare team to create and implement personalized nutrition plans for patients.
- Conduct individual and group education sessions on nutrition and healthy living.
- Monitor and assess patients' dietary needs and make necessary adjustments to their nutrition plans.
- Maintain accurate and detailed records of patients' progress and nutrition plans.
- Keep up-to-date with the latest nutrition research and trends to provide the most current and effective advice to patients.
- Communicate with patients' families and caregivers to ensure continuity of care and support.
- Adhere to all safety and sanitation guidelines when handling and preparing food.
- Assist with meal planning and preparation for patients with special dietary needs.
- Advocate for healthy living and nutrition within the community by participating in community events and initiatives.
- Collaborate with other Nutrition Assistants and healthcare professionals to continuously improve nutrition services.
- Maintain confidentiality and adhere to HIPAA regulations when handling patients' personal information.
- Provide support and guidance to patients as they make lifestyle changes to improve their overall well-being.
- Stay organized and manage time effectively to meet the needs of multiple patients.
- Continuously strive to improve patient satisfaction and outcomes through quality care and excellent customer service.
- Uphold the mission, values, and standards of UCHealth in all interactions with patients, colleagues, and the community.
Bachelor's Degree In Nutrition Or Related Field
Previous Experience In A Clinical Or Healthcare Setting
Knowledge Of Nutrition Principles And Guidelines, Including Usda Dietary Guidelines And Myplate
Strong Communication And Interpersonal Skills To Collaborate With Patients And Healthcare Team
