We are currently seeking a compassionate and experienced Nurse Practitioner to join our Specialty Care team in Palliative Care. As a Nurse Practitioner in this role, you will have the opportunity to work closely with patients facing serious or life-limiting illnesses, providing them with personalized and comprehensive care. We are looking for a driven individual with a strong background in palliative care and a passion for improving the quality of life for patients and families during difficult times.   1. Conduct comprehensive assessments of patients in the palliative care program and develop individualized care plans based on their specific needs and goals.
  2. Collaborate with interdisciplinary team members, including physicians, nurses, social workers, and chaplains, to provide holistic care to patients and families.
  3. Monitor and manage symptoms and side effects of treatments, providing evidence-based interventions and therapies to improve quality of life.
  4. Educate patients and families about their illness, treatment options, and end-of-life care planning, promoting shared decision-making and empowering patients to make informed choices.
  5. Serve as a resource and advocate for patients and families, addressing their unique physical, emotional, and spiritual needs.
  6. Coordinate and communicate with community resources and healthcare providers to facilitate continuity of care for patients transitioning between different care settings.
  7. Participate in quality improvement initiatives and research projects to advance the field of palliative care and improve patient outcomes.
  8. Ensure accurate and timely documentation of patient encounters and progress in electronic health records.
  9. Continuously update knowledge and skills through professional development activities, staying current with best practices and evidence-based guidelines in palliative care.
  10. Uphold ethical and legal standards of practice, maintaining patient confidentiality and respecting their autonomy and cultural beliefs.

Job Qualifications
  • Advanced Degree In Nursing: A Nurse Practitioner Specialty Care - Palliative Care Position At Uchealth Requires A Master's Or Doctoral Degree In Nursing, With A Focus On Palliative Care.

  • Board Certification In Palliative Care: The Ideal Candidate Should Hold Certification From The American Nurses Credentialing Center (Ancc) Or The National Board For Certification Of Hospice And Palliative Nurses (Nbchpn) In Palliative Care.

  • Experience In Palliative Care: Candidates With At Least 2-3 Years Of Experience In A Palliative Care Setting Will Be Preferred. This Experience Should Include Managing Complex Pain And Symptom Management, End-Of-Life Care, And Facilitating Difficult Conversations With Patients And Families.

  • Strong Communication And Interpersonal Skills: As A Nurse Practitioner In Palliative Care, Effective Communication With Patients, Families, And Interdisciplinary Teams Is Crucial. The Candidate Must Possess Excellent Communication And Interpersonal Skills To Provide Compassionate Care And Facilitate Difficult Conversations.

  • State Licensure And Dea Certification: The Candidate Must Be A Licensed Nurse Practitioner In The State Of Colorado And Hold A Valid Dea Certification. This Is Essential For Prescribing Controlled Substances And Managing Patient Care In Accordance With State Regulations.
