
Nurse Navigator - Oncology Care Clinic

As a Nurse Navigator, you will play a crucial role in guiding patients through their cancer treatment journey, providing support and resources along the way. - Serve as the primary point of contact for patients and their families throughout their cancer treatment journey.
- Conduct initial assessments and develop individualized care plans for patients, in collaboration with the healthcare team.
- Provide education and support to patients and their families regarding their diagnosis, treatment options, and potential side effects.
- Coordinate and schedule appointments with various healthcare providers, including oncologists, radiologists, and surgeons.
- Monitor and track patients' progress and communicate updates to the healthcare team.
- Identify and address any barriers to care, such as financial concerns or transportation issues.
- Connect patients with appropriate resources and support services, including psychological counseling, nutrition counseling, and support groups.
- Serve as a liaison between patients and healthcare providers, ensuring effective communication and coordination of care.
- Maintain accurate and up-to-date documentation of patient interactions and progress.
- Continuously evaluate and improve the Nurse Navigator program to ensure the highest quality of care for patients.
- Participate in staff meetings, training, and development opportunities to enhance knowledge and skills related to oncology care.
- Adhere to all healthcare regulations and protocols, including maintaining patient confidentiality.
- Serve as an advocate for patients' needs and concerns, and escalate any issues to the appropriate parties.
- Collaborate with community organizations and resources to expand and enhance support services for cancer patients.
- Stay informed of current developments and advancements in oncology care and incorporate them into practice.
Bachelor's Degree In Nursing (Bsn) From An Accredited Institution.
Current And Unrestricted Registered Nurse (Rn) License In The State Of Colorado.
Minimum Of 3 Years Of Experience In Oncology Nursing.
Certification In Oncology Nursing (Ocn) From The Oncology Nursing Certification Corporation (Oncc).
