  1. Serve as the primary point of contact for patients and their families throughout their cancer journey, providing compassionate and personalized care.
  2. Conduct initial assessments and collaborate with healthcare team to develop individualized care plans for each patient.
  3. Educate patients and their families on their diagnosis, treatment options, and potential side effects.
  4. Coordinate and schedule necessary appointments and procedures for patients, ensuring timely and efficient care.
  5. Act as a liaison between patients and healthcare providers, advocating for the patients' needs and concerns.
  6. Provide emotional and psychological support to patients and their families, addressing any fears or concerns they may have.
  7. Connect patients with appropriate resources and support services, such as financial assistance or support groups.
  8. Maintain accurate and thorough documentation of patient interactions and progress.
  9. Stay current on advancements in cancer treatment and resources, and share knowledge with patients and colleagues.
  10. Participate in multidisciplinary team meetings to discuss patient care and treatment plans.
  11. Collaborate with community organizations and healthcare partners to enhance patient care and support.
  12. Provide follow-up care and support to patients after treatment, ensuring continuity of care.
  13. Continuously evaluate and improve the Nurse Navigator program to enhance patient outcomes and satisfaction.
  14. Adhere to all HIPAA and patient confidentiality guidelines.
  15. Communicate effectively with patients, families, and healthcare team members to ensure a smooth and positive experience.

Job Qualifications
  • Bachelor's Degree In Nursing: A Nurse Navigator For The Cancer Service Line At Uchealth Must Possess A Bachelor's Degree In Nursing From An Accredited Institution. This Educational Background Provides A Strong Foundation In Nursing Principles And Clinical Knowledge Necessary For The Role.

  • Active Nursing License: The Candidate Must Hold An Active Nursing License In The State Of Colorado. This Ensures That They Have Met The Necessary Requirements And Are Authorized To Practice As A Nurse In This State.

  • Experience In Oncology Nursing: A Minimum Of 3-5 Years Of Experience In Oncology Nursing Is Required For This Role. This Experience Provides A Comprehensive Understanding Of Cancer Care And Treatments, As Well As The Ability To Effectively Communicate With Patients And Their Families.

  • Strong Communication And Interpersonal Skills: As A Nurse Navigator In The Cancer Service Line, The Candidate Must Have Excellent Communication Skills To Effectively Educate And Support Patients And Their Families. They Must Also Have Strong Interpersonal Skills To Collaborate With A Multidisciplinary Team And Coordinate Care For Patients.

  • Knowledge Of Cancer Care Resources: The Ideal Candidate Should Have Knowledge Of Community And National Resources Available For Cancer Patients. This Includes Understanding Insurance Coverage, Financial Assistance Programs, And Support Groups. This Knowledge Will Help The Nurse Navigator In Providing Comprehensive Care To Patients And Their Families.
