- Collaborate with the multidisciplinary team to ensure safe and effective medication therapy management for patients in the critical care/emergency medicine department.
- Monitor and review patient medication regimens to identify and resolve any drug therapy problems.
- Provide drug information and education to healthcare professionals and patients.
- Maintain accurate and complete patient medication records.
- Participate in patient rounds and provide recommendations for medication adjustments as needed.
- Conduct medication reconciliation and identify potential drug interactions.
- Stay updated on current developments and advancements in critical care and emergency medicine to provide the best possible care to patients.
- Assist in the development and implementation of protocols and guidelines related to medication use in the critical care/emergency medicine department.
- Collaborate with the pharmacy team to ensure proper storage, preparation, and dispensing of medications.
- Participate in quality improvement initiatives and contribute to the achievement of departmental and organizational goals.
- Maintain compliance with all relevant laws, regulations, and policies related to medication use.
- Serve as a resource to other healthcare professionals and provide guidance on medication-related issues.
- Actively participate in the training and development of pharmacy students and residents.
- Maintain professional competency and actively participate in continuing education opportunities.
- Demonstrate a commitment to providing compassionate and patient-centered care.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: This Is The Minimum Educational Requirement For A Clinical Pharmacist Position At Uchealth. A Pharmd Degree Ensures That The Candidate Has A Strong Foundation In Pharmacology, Pharmacotherapy, And Patient Care.
Completion Of A Pgy-2 Residency In Critical Care Or Emergency Medicine: A Post-Graduate Year 2 (Pgy-2) Residency In Either Critical Care Or Emergency Medicine Provides Specialized Training In Managing Complex Medication Regimens, Critical Care Interventions, And Emergency Response Protocols.
Board Certification In Pharmacotherapy Or Critical Care: Candidates Should Possess Board Certification In Either Pharmacotherapy (Bcps) Or Critical Care Pharmacy (Bcccp) From The Board Of Pharmacy Specialties (Bps). This Certification Demonstrates A High Level Of Expertise And Commitment To The Field.
Experience In A Critical Care Or Emergency Medicine Setting: Candidates Should Have A Minimum Of 2-3 Years Of Experience Working As A Pharmacist In A Critical Care Or Emergency Medicine Setting. This Experience Ensures That The Candidate Is Familiar With The Unique Challenges And Fast-Paced Environment Of These Specialties.
Strong Communication And Teamwork Skills: The Candidate Should Possess Excellent Communication And Teamwork Skills To Effectively Collaborate With Other Healthcare Professionals In A Critical Care Or Emergency Medicine Setting. This Includes The Ability To Provide Clear And Concise Medication Recommendations And Participate In Interprofessional Rounds And Meetings.
