  1. Facilitate effective communication among healthcare teams: This includes communicating important patient information, treatment plans, and updates to all members of the healthcare team, including physicians, nurses, therapists, and other healthcare professionals.
  2. Coordinate care for patients: Work closely with patients, their families, and healthcare providers to ensure that all aspects of a patient's care are properly coordinated and integrated.
  3. Advocate for patients: Serve as a patient advocate and liaison, ensuring that their needs and concerns are addressed and that they receive the best possible care.
  4. Navigate complex healthcare systems: Understand and navigate the complexities of the healthcare system, including insurance, referrals, and other administrative processes.
  5. Ensure comprehensive and compassionate care: Collaborate with healthcare teams to create and implement care plans that prioritize the physical, emotional, and social well-being of patients.
  6. Maintain accurate records: Keep detailed and accurate records of patient information, treatment plans, and communication between healthcare providers.
  7. Stay up-to-date on healthcare regulations and policies: Keep informed of changes in healthcare regulations, policies, and procedures to ensure compliance and provide the best care for patients.
  8. Collaborate with community resources: Work with community resources to provide patients with access to additional support services, such as financial assistance or transportation.
  9. Monitor and evaluate patient progress: Regularly monitor and evaluate patient progress and adjust care plans as needed to ensure the best outcomes.
  10. Provide education and support: Educate patients and their families on their healthcare options, treatment plans, and resources available to them. Offer support and guidance throughout the care coordination process.

Job Qualifications
