
Transition Coordinator, CCMTP

In this role, you will play a crucial role in assisting clients with mental health challenges in their transition from institutional care to community-based services. - Coordinate transitional care services for clients with mental health challenges, including developing and implementing individualized care plans.
- Collaborate with mental health providers, community organizations, and other resources to ensure clients have access to necessary support and services.
- Advocate for clients and their needs, including navigating and accessing resources and services.
- Conduct assessments and evaluations to determine clients' needs and goals.
- Monitor and evaluate clients' progress and make adjustments to care plans as needed.
- Provide crisis intervention and support to clients as needed.
- Maintain accurate and up-to-date client records and documentation.
- Facilitate communication and coordination between clients, their families, and other involved parties.
- Educate clients on self-care strategies and techniques to promote independence and recovery.
- Attend team meetings and collaborate with other team members to ensure comprehensive and effective care for clients.
- Stay current on mental health trends, resources, and best practices to inform care plans and services.
- Adhere to all ethical and professional standards in working with clients.
- Provide support and guidance to clients in navigating the healthcare system and accessing appropriate services.
- Participate in training and professional development opportunities to enhance knowledge and skills.
- Perform other duties as assigned by the supervisor.
Bachelor's Degree In Psychology, Social Work, Or Related Field
Minimum Of 2 Years Experience Working With Individuals With Mental Illness And/Or Substance Use Disorders
Knowledge Of Community Resources And Ability To Coordinate Services For Clients
Excellent Communication And Organizational Skills
Certification In Certified Community Mental Health Transition Practitioner (Ccmtp) Or Willingness To Obtain Within 6 Months Of Employment
