- Develop and implement individualized transition plans for clients with mental illness and developmental disabilities.
- Work closely with clients, their families, and a multidisciplinary team to identify goals and resources for successful community integration.
- Coordinate and manage care for vulnerable populations, including scheduling appointments, connecting clients with necessary services, and tracking progress.
- Advocate for clients' needs and rights, ensuring they receive appropriate support and services.
- Conduct regular assessments to monitor clients' progress and adjust transition plans as needed.
- Collaborate with community organizations and resources to provide clients with a range of support options.
- Maintain accurate and up-to-date documentation of clients' transition plans and progress.
- Communicate effectively with clients, families, team members, and external partners.
- Stay current on best practices and regulations related to mental health and disability services.
- Demonstrate empathy, compassion, and patience towards clients and their families.
- Uphold ethical standards and maintain confidentiality in all interactions with clients.
- Attend team meetings and participate in case conferences to provide updates and receive feedback.
- Continuously evaluate and improve transition services to meet the evolving needs of clients.
- Serve as a positive and professional representative of the organization in all interactions.
- Adhere to all policies and procedures of the organization.
Bachelor's Degree In Social Work, Psychology, Or A Related Field.
Previous Experience Working With Individuals With Mental Illness, Preferably In A Community-Based Setting.
Strong Understanding Of The Transition Process And Ability To Develop Individualized Plans For Clients.
Excellent Communication And Organizational Skills.
Ability To Work Collaboratively With A Multidisciplinary Team And Coordinate Services With Community Partners.
