Job Description

Do you have experience in coordinating and managing care for vulnerable populations? If so, then Thresholds is looking for you to join our team as a Transition Coordinator. We are seeking a highly motivated and compassionate individual to support our clients in their journey towards greater independence and self-sufficiency. In this role, you will work closely with our clients, their families, and a multidisciplinary team to develop and implement transition plans that promote successful community integration.   1. Develop and implement individualized transition plans for clients with mental illness and developmental disabilities.
  2. Work closely with clients, their families, and a multidisciplinary team to identify goals and resources for successful community integration.
  3. Coordinate and manage care for vulnerable populations, including scheduling appointments, connecting clients with necessary services, and tracking progress.
  4. Advocate for clients' needs and rights, ensuring they receive appropriate support and services.
  5. Conduct regular assessments to monitor clients' progress and adjust transition plans as needed.
  6. Collaborate with community organizations and resources to provide clients with a range of support options.
  7. Maintain accurate and up-to-date documentation of clients' transition plans and progress.
  8. Communicate effectively with clients, families, team members, and external partners.
  9. Stay current on best practices and regulations related to mental health and disability services.
  10. Demonstrate empathy, compassion, and patience towards clients and their families.
  11. Uphold ethical standards and maintain confidentiality in all interactions with clients.
  12. Attend team meetings and participate in case conferences to provide updates and receive feedback.
  13. Continuously evaluate and improve transition services to meet the evolving needs of clients.
  14. Serve as a positive and professional representative of the organization in all interactions.
  15. Adhere to all policies and procedures of the organization.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Previous Experience Working With Individuals With Mental Illness, Preferably In A Community-Based Setting.

  • Strong Understanding Of The Transition Process And Ability To Develop Individualized Plans For Clients.

  • Excellent Communication And Organizational Skills.

  • Ability To Work Collaboratively With A Multidisciplinary Team And Coordinate Services With Community Partners.
