Team Leader, Service Integration

Responsibilities:

1. Develop and implement service integration systems that comply with applicable state and federal regulations.
2. Lead, mentor, and support a team of social service professionals.
3. Establish collaborative relationships with other departments in the organization.
4. Monitor and evaluate service integration systems to ensure effectiveness and efficiency.
5. Develop strategies to increase service integration success.
6. Create and maintain accurate and up-to-date records of service integration programs.
7. Facilitate meetings with internal and external stakeholders.
8. Provide support and guidance to individuals and families in need.
9. Develop training programs for staff and stakeholders.
10. Monitor and evaluate team performance.
