Job Description

We are currently seeking a highly motivated and skilled individual to join our team as the Team Leader for Quality Assurance and Care Coordination. As a Team Leader, you will play a crucial role in ensuring the delivery of exceptional care to our clients and maintaining the highest standards of quality assurance.   1. Oversee the quality assurance and care coordination processes for all programs and services provided by Thresholds.
  2. Develop and implement policies and procedures to ensure compliance with all relevant regulations and standards.
  3. Monitor and analyze data to identify trends and areas for improvement in the delivery of care.
  4. Collaborate with program directors and staff to develop and implement quality improvement initiatives.
  5. Conduct regular audits and reviews to ensure the accuracy and completeness of program documentation.
  6. Train and mentor staff on quality assurance practices and best practices for delivering high-quality care.
  7. Coordinate with external agencies and partners to ensure compliance with contractual requirements and quality standards.
  8. Serve as a liaison between different departments to facilitate effective communication and collaboration.
  9. Identify and address any potential risks or issues that may impact the quality of care provided.
  10. Lead and participate in performance improvement and quality assurance committees.
  11. Develop and maintain relationships with stakeholders and community partners to enhance the quality of care provided.
  12. Stay updated on industry trends and best practices in quality assurance and care coordination.
  13. Serve as a role model for staff by demonstrating a commitment to the mission and values of Thresholds.
  14. Handle and address any concerns or complaints related to quality of care.
  15. Prepare and present reports on quality assurance and care coordination to senior leadership.

Job Qualifications
  • Bachelor's Degree In A Related Field Such As Social Work, Psychology, Or Healthcare Administration.

  • Minimum Of 3 Years Of Experience In A Leadership Role, Preferably In A Healthcare Or Social Services Setting.

  • Strong Knowledge Of Quality Assurance And Care Coordination Principles And Practices.

  • Experience Working With Individuals With Mental Health And/Or Substance Abuse Disorders.

  • Excellent Communication, Problem-Solving, And Organization Skills.
