As a Part-Time Community Support Specialist for our Homeless Outreach Program, you will play a vital role in connecting with and assisting those in need. This is a unique opportunity to make a meaningful impact in the lives of vulnerable individuals and contribute to the fight against homelessness in our community. We are seeking a dedicated and empathetic individual with strong communication and outreach skills to join our team.   1. Develop and maintain relationships with individuals experiencing homelessness in the community.
  2. Conduct outreach and engage with individuals in various settings, such as shelters, parks, and streets.
  3. Provide support and resources to individuals to meet their basic needs, including food, shelter, and healthcare.
  4. Collaborate with other community organizations and agencies to connect individuals to additional services and resources.
  5. Conduct assessments and develop individualized care plans for each client.
  6. Monitor progress and provide ongoing support to clients to achieve their goals.
  7. Advocate for clients and assist them in navigating systems and accessing necessary services.
  8. Document all client interactions and maintain accurate and up-to-date records.
  9. Attend team meetings and participate in program development and improvement.
  10. Stay informed about current issues and resources related to homelessness and share information with clients and colleagues.
  11. Adhere to all agency policies and procedures and maintain professional boundaries with clients.
  12. Uphold the values of compassion, empathy, and respect in all interactions with clients.
  13. Continuously seek out opportunities for personal and professional growth to better serve clients.
  14. Represent the agency in a positive manner and promote its mission and services within the community.
  15. Perform any other duties or tasks as assigned by the supervisor.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Minimum Of 2 Years Of Experience Working With Individuals Experiencing Homelessness Or In A Similar Role.

  • Knowledge Of Community Resources And Agencies That Serve Individuals Experiencing Homelessness.

  • Strong Communication And Interpersonal Skills To Effectively Engage And Build Rapport With Clients.

  • Ability To Work Independently And As Part Of A Team, With A Flexible Schedule To Accommodate The Needs Of Clients.
