1. Provide support to the 2025 Community Mental Health program by assisting with program planning and implementation.
2. Collaborate with team members to develop and implement individualized treatment plans for clients.
3. Conduct intake assessments and screenings to determine client needs and eligibility for services.
4. Facilitate group therapy sessions and individual counseling sessions.
5. Assist with crisis intervention and management for clients in acute distress.
6. Maintain accurate and up-to-date client records and documentation.
7. Participate in regular team meetings to discuss client progress and treatment planning.
8. Assist with outreach efforts to raise awareness about mental health services and resources.
9. Provide support and assistance to clients in navigating the mental health system and connecting them to appropriate resources.
10. Attend trainings and workshops to expand knowledge and skills in the mental health field.
11. Uphold the values and mission of Thresholds in all interactions with clients and the community.
12. Adhere to all ethical and legal guidelines when working with clients.
13. Communicate effectively with clients, team members, and other professionals.
14. Maintain confidentiality of all client information and adhere to HIPAA guidelines.
15. Perform other duties as assigned by the supervisor.
