1. Greet and welcome individuals seeking mental health services with empathy and compassion.
2. Conduct intake assessments to determine the specific needs of each individual and their eligibility for services.
3. Educate individuals on the services and resources available at Thresholds.
4. Maintain accurate and up-to-date records of all intake information.
5. Collaborate with other team members to develop and implement individualized treatment plans.
6. Provide referrals to appropriate mental health professionals and community resources.
7. Ensure timely and efficient processing of all intake paperwork.
8. Follow all legal and ethical standards in handling confidential information.
9. Continuously stay updated on mental health issues, resources, and services.
10. Build and maintain relationships with community partners to expand referral network.
11. Communicate effectively with individuals, their families, and other stakeholders.
12. Adhere to all policies, procedures, and protocols of Thresholds.
13. Attend trainings and meetings as required.
14. Work collaboratively with other team members to ensure a seamless intake process.
15. Act as an advocate for individuals and their families to ensure they receive the best possible care.
