Job Description

Welcome to Thresholds, a leading mental health organization dedicated to providing support and resources to individuals in need. We are currently seeking a compassionate and dedicated Community Support Specialist to join our team on the West Side. As a Community Support Specialist, you will play a crucial role in helping individuals with mental illness live independently and achieve their goals. We are looking for someone who is passionate about making a positive impact in the community and has experience working with individuals with mental illness.   1. Develop and maintain relationships with individuals with mental illness in the community.
  2. Provide emotional support and guidance to individuals in need.
  3. Assist clients in setting and achieving their goals for independent living.
  4. Collaborate with other team members to develop and implement individualized treatment plans for clients.
  5. Advocate for clients' rights and needs within the community.
  6. Connect clients with resources and services such as housing, employment, and healthcare.
  7. Monitor and assess clients' progress and make necessary adjustments to treatment plans.
  8. Provide crisis intervention and support as needed.
  9. Maintain accurate and confidential client records and documentation.
  10. Attend and actively participate in team meetings and trainings.
  11. Stay up-to-date on current mental health practices and trends.
  12. Communicate effectively and professionally with clients, colleagues, and community partners.
  13. Adhere to all agency policies and procedures.
  14. Promote a positive and inclusive work environment that embraces diversity.
  15. Demonstrate a commitment to the mission and values of Thresholds.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Knowledge Of Community Resources And Ability To Connect Clients With Appropriate Services.

  • Minimum Of 2 Years Of Experience Working With Individuals With Mental Illness Or Developmental Disabilities.

  • Strong Communication And Problem-Solving Skills.
