Job Description

If so, Thresholds is seeking a compassionate and dedicated Community Support Specialist to join our team and make a positive impact in the lives of our clients. As a member of our Community Mental Health - Peer Program, you will work closely with individuals to promote recovery, independence, and community integration.   1. Provide direct support and assistance to individuals with mental health challenges in a compassionate and empathetic manner.
  2. Utilize personal experience with mental illness to connect with and understand the needs of clients.
  3. Serve as a role model and advocate for clients, promoting recovery and independence.
  4. Collaborate with clients to develop individualized treatment plans and goals.
  5. Conduct regular check-ins and assessments to monitor progress and make necessary adjustments to treatment plans.
  6. Assist clients in accessing and navigating community resources and services.
  7. Educate clients and their families on mental health and wellness strategies.
  8. Foster a safe and inclusive environment for clients to share their experiences and challenges.
  9. Document and maintain accurate records of client progress and interactions.
  10. Participate in team meetings and case conferences to discuss client progress and coordinate care.
  11. Stay up-to-date on current mental health research and best practices.
  12. Provide crisis intervention and support as needed.
  13. Serve as a liaison between clients and other mental health professionals or agencies.
  14. Advocate for clients' rights and needs within the community.
  15. Adhere to all ethical and confidentiality guidelines.
  16. Participate in ongoing training and professional development opportunities.
  17. Maintain a positive and professional attitude at all times.
  18. Continuously strive to improve the quality of services provided to clients.
  19. Collaborate with other members of the team to ensure a comprehensive and holistic approach to client care.
  20. Embody the mission and values of Thresholds in all interactions and work performed.

Job Qualifications
  • Bachelor's Degree In Psychology, Social Work, Or A Related Field.

  • Previous Experience Working With Individuals With Mental Health Conditions.

  • Certification As A Peer Support Specialist Or Willingness To Obtain Certification Within 6 Months Of Hire.

  • Familiarity With Community Mental Health Resources And Services.

  • Strong Communication And Interpersonal Skills, Including The Ability To Establish Rapport With Clients And Work Effectively In A Team Setting.
