- Provide direct support and assistance to individuals facing mental health challenges.
- Assist with the development and implementation of individualized support plans.
- Collaborate with other members of the team to ensure a comprehensive approach to care.
- Utilize a person-centered and strengths-based approach to support individuals in achieving their goals.
- Build and maintain positive relationships with clients, families, and community resources.
- Advocate for clients' rights and needs within the community.
- Monitor and document clients' progress and any changes in their mental health status.
- Provide crisis intervention and de-escalation as needed.
- Participate in team meetings and case conferences to discuss clients' needs and progress.
- Stay up-to-date on best practices and developments in the mental health field.

- Communicate effectively and professionally with clients, colleagues, and external stakeholders.
- Attend trainings and workshops to enhance knowledge and skills.

- Collaborate with community partners to provide resources and referrals for clients.
- Assist clients with daily living skills, including housing, employment, and education.
- Maintain accurate and timely documentation of services provided.
- Adhere to all regulations and guidelines related to client care.

Bachelor's Degree In Social Work, Psychology, Or A Related Field: A Community Support Specialist At Thresholds Should Have A Strong Educational Background In A Field Related To Mental Health And Social Work.
Experience Working With Individuals With Mental Illness: This Role Requires Experience Working With Individuals Who Have Mental Health Concerns, Including Knowledge Of Various Treatment Methods And An Understanding Of The Challenges Faced By This Population.
Excellent Communication And Interpersonal Skills: As A Community Support Specialist, You Will Be Working Closely With Clients, Their Families, And Other Mental Health Professionals. Strong Communication And Interpersonal Skills Are Essential For Building Rapport And Providing Effective Support.
Knowledge Of Community Resources And Social Services: Thresholds Aims To Connect Clients With The Resources They Need To Live Independently And Improve Their Quality Of Life. A Community Support Specialist Should Have A Strong Understanding Of Community Resources And Social Services Available To Individuals With Mental Illness.
Ability To Work Independently And As Part Of A Team: This Role Will Involve Both Independent Work With Clients And Collaboration With A Team Of Mental Health Professionals. A Successful Community Support Specialist Should Be Able To Work Autonomously And As Part Of A Team To Provide The Best Support For Clients.
