Job Description

We are seeking a compassionate and dedicated Case Manager to join our team and make a difference in the lives of young adults in our Youth Transitional Living Program. As a Case Manager, you will have the opportunity to provide support and guidance to youth as they navigate the challenges of transitioning into adulthood.   1. Facilitate a supportive and empowering environment for young adults in the Youth Transitional Living Program.
  2. Provide case management services to assigned clients, including conducting assessments, creating and implementing individualized service plans, and monitoring progress towards goals.
  3. Collaborate with clients, their families, and other stakeholders to develop and maintain a comprehensive support network for each individual.
  4. Advocate for clients’ needs and rights, and assist them in accessing resources and services to meet those needs.
  5. Conduct regular check-ins and provide ongoing support and guidance to clients, including addressing any crises or challenges that may arise.
  6. Maintain accurate and timely documentation of client progress and services provided in accordance with agency policies and procedures.
  7. Coordinate and facilitate group activities and workshops to promote life skills development, healthy relationships, and positive decision-making.
  8. Attend regular team meetings and participate in case conferences to discuss client progress and develop strategies for improvement.
  9. Keep current with community resources and services available to young adults, and make appropriate referrals as needed.
  10. Adhere to ethical standards and maintain confidentiality in all interactions with clients and their information.
  11. Collaborate with other agency staff and community partners to ensure a coordinated and holistic approach to supporting clients.
  12. Continuously evaluate and assess the effectiveness of services provided and make recommendations for improvement.
  13. Act as a positive role model for clients, demonstrating professionalism, empathy, and respect in all interactions.
  14. Participate in trainings and professional development opportunities to enhance knowledge and skills related to case management and working with youth.
  15. Promote the mission and values of Thresholds and represent the organization in a positive and professional manner at all times.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Or A Related Field.

  • Minimum Of 2 Years Of Experience Working With At-Risk Youth Or In A Related Field.

  • Knowledge Of Best Practices In Transitional Living Programs For Youth.

  • Familiarity With Mental Health And Substance Abuse Issues And Ability To Provide Appropriate Support And Referrals.

  • Strong Communication And Organizational Skills, Including The Ability To Work Collaboratively With Other Team Members And External Agencies.
