- Provide comprehensive support and services to young adults struggling with mental health challenges.
- Serve as a compassionate and driven Case Manager for the Young Adult Program.
- Make a positive impact in the lives of young adults by providing personalized care and guidance.
- Utilize experience in mental health to effectively support and advocate for clients.
- Collaborate with other members of the team to develop and implement individualized treatment plans.
- Conduct regular assessments and evaluations of clients' mental health needs.
- Coordinate and facilitate access to necessary resources and community services.
- Monitor and document clients' progress and make necessary adjustments to treatment plans.
- Communicate effectively with clients, their families, and other healthcare professionals.
- Maintain accurate and confidential client records in accordance with agency policies and regulations.
- Participate in ongoing training and professional development opportunities.
- Adhere to all ethical and legal standards related to client care and confidentiality.
- Act as an advocate for clients and promote their rights and interests.
- Continuously evaluate the effectiveness of services and make recommendations for improvement.
- Demonstrate a strong commitment to the mission and values of Thresholds.
Bachelor's Degree In Social Work, Psychology, Or A Related Field
Minimum Of 2 Years Experience Working With Young Adults In A Mental Health Or Social Services Setting
Knowledge Of And Experience With Evidence-Based Practices For Young Adults With Mental Illness, Such As Supported Employment And Supported Education
Strong Understanding Of The Challenges Facing Young Adults With Mental Illness, Including Housing Instability, Employment Barriers, And Social Isolation
Ability To Build Rapport And Establish Trust With Clients, And Effectively Communicate And Collaborate With A Multi-Disciplinary Team To Develop And Implement Individualized Treatment Plans.
