Job Description

In this role, you will play a crucial role in guiding patients through their healthcare journey, providing support and resources to ensure they receive the best possible care.   1. Serve as the primary point of contact for patients, providing guidance and support throughout their healthcare journey.
  2. Build relationships with patients to understand their unique needs and develop personalized care plans.
  3. Assist patients in navigating the complex healthcare system and educate them on available resources and services.
  4. Collaborate with healthcare providers to ensure seamless coordination of care for patients.
  5. Conduct thorough assessments of patients' physical, emotional, and social needs.
  6. Provide emotional support and counseling to patients and their families during difficult times.
  7. Advocate for patients' rights and ensure they receive quality care.
  8. Monitor patients' progress and make necessary adjustments to their care plans.
  9. Maintain accurate and up-to-date documentation of patient interactions and progress.
  10. Participate in team meetings and collaborate with other healthcare professionals to improve the patient experience.
  11. Continuously stay updated on healthcare policies, regulations, and resources to better assist patients.
  12. Uphold the ethical and professional standards of the healthcare industry in all interactions with patients and colleagues.

Job Qualifications
  • Bachelor's Degree In Healthcare Management, Social Work, Or A Related Field.

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Patient Advocacy Or Navigation Roles.

  • Comprehensive Knowledge Of The Healthcare System And Insurance Processes.

  • Strong Communication And Interpersonal Skills To Effectively Collaborate With Patients, Healthcare Providers, And Community Organizations.

  • Familiarity With Electronic Medical Records And Ability To Navigate And Document Patient Information Accurately.
