Job Description

Are you passionate about helping individuals struggling with substance abuse? Do you have a strong background in mental health and addiction treatment? If so, we have an exciting opportunity for you to join our team as a Substance Use Navigator (SUN) at Sutter Health. As a SUN, you will play a crucial role in guiding patients through their recovery journey and connecting them with the necessary resources and support. We are looking for a highly empathetic and driven individual with a minimum of 3 years of experience in substance abuse treatment and a Bachelor's degree in a related field. Join us in making a positive impact on the lives of those affected by addiction.

  1. Conduct initial assessments and develop personalized treatment plans for patients struggling with substance abuse.
  2. Provide individual and group counseling sessions for patients to address their addiction and mental health needs.
  3. Collaborate with other healthcare professionals, such as doctors and therapists, to ensure a comprehensive treatment approach for patients.
  4. Utilize evidence-based practices and stay up to date on the latest research in substance abuse treatment to provide the best care for patients.
  5. Advocate for patients and their families to ensure they receive the appropriate resources and support for their recovery journey.
  6. Connect patients with community resources, such as support groups and sober living homes, to aid in their recovery and maintain sobriety.
  7. Keep accurate and confidential records of patient progress and treatment plans.
  8. Educate patients and their families on addiction and the recovery process, including relapse prevention strategies.
  9. Create a safe and supportive environment for patients to address their addiction and mental health challenges.
  10. Participate in regular team meetings and case consultations to discuss patient progress and treatment plans.
  11. Continuously evaluate and adjust treatment plans to meet the changing needs of patients.
  12. Adhere to ethical and legal standards in all aspects of the job, including patient confidentiality and boundaries.
  13. Stay informed about insurance coverage and reimbursement processes to assist patients in accessing necessary treatment services.
  14. Attend trainings and conferences to further develop knowledge and skills in substance abuse treatment.
  15. Remain compassionate and non-judgmental towards patients while promoting a sense of hope and empowerment in their recovery journey.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Counseling, Or A Related Field.

  • Certified Alcohol And Drug Counselor (Cadc) Certification Or Equivalent.

  • Minimum Of 2 Years Of Experience Working With Individuals Struggling With Substance Use Disorders.

  • Knowledge Of Evidence-Based Treatment Approaches And Resources For Substance Use Disorders.

  • Excellent Communication And Interpersonal Skills To Effectively Engage With Patients, Families, And Healthcare Professionals.
