- Conduct comprehensive assessments to identify the specific needs and goals of each patient and develop individualized treatment plans.
- Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure coordinated and holistic care for patients.
- Monitor and document patient progress, adjust treatment plans as needed, and communicate updates to the patient, their family, and the healthcare team.
- Educate patients and their families on techniques and strategies to improve speech and language skills and manage any swallowing difficulties.
- Maintain accurate and timely patient records and comply with all legal and ethical standards, as well as Sutter Health policies and procedures.
- Stay current with advancements in the field of speech therapy and participate in continuing education to enhance knowledge and skills.
- Maintain a compassionate and supportive approach to patient care, understanding the unique challenges and needs of each individual.
- Adhere to all safety protocols and infection control measures to ensure the health and well-being of patients and staff.

Master's Degree In Speech-Language Pathology Or Communication Disorders From An Accredited University.
Certification Of Clinical Competence In Speech-Language Pathology (Ccc-Slp) From The American Speech-Language-Hearing Association (Asha).
Current State Licensure As A Speech-Language Pathologist In California.
Minimum Of 2 Years Of Experience In Home Health Or Related Field, With A Focus On Adult And Geriatric Populations.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With Patients, Families, And Other Healthcare Professionals.
