- Conduct comprehensive assessments of patients' speech and language abilities in a home health setting.
- Develop individualized treatment plans to address patients' specific needs and goals.
- Provide evidence-based speech and language therapy services to patients in their homes.
- Monitor and evaluate patients' progress, adjusting treatment plans as needed.
- Collaborate with interdisciplinary team members, including physicians, nurses, and other therapists, to coordinate patient care.
- Educate patients and their families on speech and language disorders and strategies for improving communication.
- Maintain accurate and timely documentation of patient evaluations, treatments, and progress.
- Adhere to ethical and professional standards in accordance with state licensing regulations and company policies.
- Stay current on advancements and best practices in speech and language therapy through continuing education and professional development.
- Demonstrate empathy, compassion, and patience in working with patients and their families.
- Display excellent communication skills, both written and verbal, to effectively collaborate with team members and communicate with patients.
- Provide exceptional customer service and represent Sutter Health positively in the community.
- Adhere to all safety and infection control policies and procedures.
- Maintain a flexible schedule to accommodate patient needs and meet productivity standards.

Master's Degree In Speech-Language Pathology From An Accredited University.
Valid State License To Practice Speech-Language Pathology.
Minimum Of 2 Years Of Experience In Home Health Or Related Setting.
Asha Certification Of Clinical Competence In Speech-Language Pathology (Ccc-Slp).
Strong Knowledge Of Speech And Language Disorders, Assessment Techniques, And Treatment Methods For Patients Of All Ages.
