  1. Providing compassionate and high-quality care to patients and their families during their end-of-life journey.
  2. Conducting comprehensive assessments and developing individualized care plans for patients.
  3. Administering medications and treatments as prescribed by physicians.
  4. Collaborating with interdisciplinary team members to ensure coordinated and effective care.
  5. Monitoring and evaluating patient progress and making necessary adjustments to care plans.
  6. Educating patients and their families on end-of-life care and providing emotional support.
  7. Communicating effectively with patients, families, and team members to ensure continuity of care.
  8. Documenting all patient care activities accurately and timely.
  9. Adhering to all legal and ethical standards in patient care.
  10. Maintaining a safe and clean work environment.
  11. Participating in ongoing education and training to stay updated on hospice care practices.
  12. Identifying and addressing any patient or family concerns or complaints.
  13. Adhering to all policies, procedures, and protocols of Sutter Health.
  14. Demonstrating compassion and empathy towards patients and their families.
  15. Promoting a positive and inclusive work environment for all team members.
  16. Maintaining a professional demeanor at all times.
  17. Participating in quality improvement initiatives and implementing changes to enhance patient care.
  18. Maintaining confidentiality of all patient information.
  19. Adhering to all infection control and safety protocols.
  20. Upholding the mission and values of Sutter Health in all job duties.

Job Qualifications
  • Current Basic Life Support (Bls) Certification

  • Bachelor's Degree In Nursing From An Accredited Institution

  • Valid Registered Nurse (Rn) License In The State Of California

  • Minimum Of 2 Years Of Experience In Hospice Or Palliative Care

  • Excellent Communication And Interpersonal Skills, With The Ability To Provide Compassionate Care To Patients And Families During End-Of-Life Situations.
