  1. Provide comprehensive and compassionate care to patients in their homes.
  2. Collaborate with a multidisciplinary team to develop individualized care plans for patients.
  3. Conduct thorough assessments of patients' physical, emotional, and social needs.
  4. Administer medications and treatments as prescribed by physicians.
  5. Monitor patients' progress and make necessary adjustments to care plans.
  6. Educate patients and their families on disease management, self-care techniques, and medication management.
  7. Ensure compliance with Medicare guidelines and regulations.
  8. Maintain accurate and up-to-date patient records.
  9. Communicate effectively with patients, families, and healthcare team members.
  10. Provide emotional support and counseling to patients and their families.
  11. Identify and address any barriers to patient care.
  12. Act as an advocate for patients' needs and rights.
  13. Participate in quality improvement initiatives and implement evidence-based practices.
  14. Maintain a current RN license and stay up-to-date with healthcare regulations and best practices.
  15. Demonstrate strong organizational and time-management skills to effectively manage a caseload of patients.
  16. Collaborate with community resources to ensure patients have access to necessary services.
  17. Continuously seek opportunities for professional growth and development.
  18. Uphold the values and mission of Sutter Health in all interactions with patients, families, and team members.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field
