1. Coordinate and provide comprehensive care to patients receiving home health services in Amador County.
2. Conduct assessments and develop individualized care plans for patients based on their specific needs and goals.
3. Collaborate with interdisciplinary team members, including physicians, therapists, and social workers, to ensure continuity and quality of care.
4. Monitor and evaluate patient progress and adjust care plans accordingly.
5. Educate patients and their families on disease management, medication management, and other aspects of their care.

13. Serve as a resource and mentor to other team members.
