We are currently seeking a compassionate and dedicated Registered Nurse to join our team as a Case Manager for our Hospice program.As a Case Manager, you will play a vital role in ensuring that our patients receive the highest quality of care during their end-of-life journey. You will work closely with a multidisciplinary team to develop and implement individualized care plans, provide hands-on nursing care, and offer emotional and spiritual support to patients and families.To excel in this role, you must hold a current Registered Nurse license and have a minimum of two years of experience in hospice or home health care.   1. Provide compassionate and dedicated care to patients and their families during their end-of-life journey.
  2. Work collaboratively with a multidisciplinary team to develop and implement individualized care plans for patients.
  3. Conduct thorough assessments of patients' physical, emotional, and spiritual needs.
  4. Provide hands-on nursing care, including administering medications, monitoring vital signs, and managing symptoms.
  5. Offer emotional and spiritual support to patients and families, providing a safe and comforting environment.
  6. Communicate effectively with patients, families, and healthcare team members to ensure continuity of care.
  7. Utilize critical thinking skills to identify and address any potential issues or concerns related to patient care.
  8. Maintain accurate and detailed patient records, documenting all care and interventions provided.
  9. Adhere to all regulatory standards and guidelines, including those set by Sutter Health and other governing bodies.
  10. Continuously strive to improve and enhance the quality of care provided to patients and their families.
  11. Participate in ongoing education and training opportunities to stay current on best practices and advancements in hospice care.

Job Qualifications
  • Bachelor's Degree In Nursing Or Related Field

  • Active Rn License In The State Of California

  • Minimum Of 2 Years Of Experience As A Case Manager In A Hospice Setting

  • Certification In Hospice And Palliative Care (Chpn) Preferred

  • Strong Communication And Organizational Skills, With The Ability To Work Independently And As Part Of A Multidisciplinary Team.
