
Primary Care Physician, PACE
Welcome to Sutter Health's PACE team! We are currently seeking a compassionate and dedicated Primary Care Physician to join our team and provide exceptional care to our PACE participants. As a Primary Care Physician, you will play a vital role in the overall health and well-being of our elderly patients. Your expertise and passion for geriatric care will be valued and appreciated by our team and the community we serve. Join us in our mission to improve the quality of life for our PACE participants. To qualify for this role, you must have a medical degree from an accredited institution and be board certified in Internal Medicine or Family Medicine. Additionally, you must hold a valid state medical license and have at least 2 years of experience providing primary care to elderly patients. A strong understanding of geriatric medicine, chronic disease management, and Medicare/Medicaid regulations is also required. - Provide exceptional primary care to elderly patients enrolled in our PACE program.
- Maintain patient records and ensure accurate and up-to-date documentation.
- Conduct thorough medical examinations, interpret diagnostic tests, and make diagnoses.
- Develop and implement individualized treatment plans for each patient.
- Work collaboratively with interdisciplinary team members to coordinate and manage patient care.
- Stay up-to-date on current medical practices, treatment options, and Medicare/Medicaid regulations.
- Communicate effectively with patients, their families, and other healthcare professionals.
- Educate patients and their families on disease prevention and management, medication adherence, and lifestyle modifications.
- Participate in quality improvement initiatives to enhance the overall care provided to our PACE participants.
- Adhere to all ethical and legal standards, including maintaining patient confidentiality.
- Serve as a mentor and resource for medical students and other staff members.
- Maintain a valid state medical license and fulfill all continuing education requirements.
- Collaborate with external healthcare providers and community resources to ensure continuity of care for patients.
- Demonstrate a commitment to providing patient-centered care and promoting the overall well-being of our PACE participants.
- Adhere to all policies and procedures set forth by Sutter Health's PACE program.
Medical Degree: A Primary Care Physician At Sutter Health Must Have A Medical Degree From An Accredited Medical School.
Board Certification: The Ideal Candidate Should Be Board Certified In Internal Medicine Or Family Medicine, Demonstrating A High Level Of Expertise In Their Specialty.
Previous Experience: A Minimum Of 3-5 Years Of Experience In Primary Care Practice Is Preferred For This Role, With A Strong Track Record Of Providing High-Quality Care To Patients.
Knowledge Of Geriatric Care: Pace (Program Of All-Inclusive Care For The Elderly) Focuses On Providing Comprehensive Care For Older Adults. Therefore, Candidates Should Have A Strong Understanding Of Geriatric Care And Be Able To Manage Complex Medical Conditions In This Population.
Communication And Interpersonal Skills: As A Primary Care Physician, Effective Communication And Interpersonal Skills Are Essential To Establish A Trusting Relationship With Patients And Their Families. The Ability To Collaborate With A Multidisciplinary Team Is Also Crucial For Success In This Role.
