  1. Provide high-quality nursing care to patients in their homes, ensuring their physical and emotional well-being.
  2. Collaborate with a multidisciplinary team to develop and implement individualized care plans for patients.
  3. Administer medications and treatments as prescribed by physicians.
  4. Monitor and assess patient's condition, report any changes or concerns to the healthcare team.
  5. Educate patients and their families on proper self-care techniques and disease management.
  6. Maintain accurate and up-to-date patient records and documentation.
  7. Maintain professional and compassionate communication with patients, families, and caregivers.
  8. Adhere to all nursing standards and procedures while ensuring patient safety and confidentiality.
  9. Participate in team meetings and case conferences to discuss patient progress and develop care plans.
  10. Continuously strive to improve nursing skills and knowledge through ongoing education and training.
  11. Follow all HIPAA regulations and maintain confidentiality of patient information.
  12. Adhere to all Sutter Health policies and procedures.
  13. Provide support and assistance to other members of the healthcare team as needed.
  14. Represent Sutter Health in a professional and positive manner at all times.
  15. Serve as an advocate for patients and their families, ensuring their needs and concerns are addressed.
  16. Stay updated on industry trends and advancements in healthcare to provide the best possible care to patients.
  17. Maintain a positive and compassionate attitude towards patients, families, and colleagues.
  18. Comply with all safety and infection control standards.
  19. Maintain a current California LVN license and adhere to all state regulations.
  20. Demonstrate a genuine passion for providing personalized care and making a positive impact on the lives of patients.

Job Qualifications
  • Graduation From An Accredited Vocational Nursing Program: A Primary Qualification For An Lvn Position At Sutter Health Would Be The Successful Completion Of An Accredited Vocational Nursing Program. This Ensures That The Candidate Has Received The Necessary Education And Training To Perform The Duties Of An Lvn In A Home Health Setting.

  • Valid And Current Lvn License: Another Key Requirement For An Lvn Position At Sutter Health Would Be A Valid And Current Lvn License From The State In Which The Candidate Will Be Practicing. This License Serves As Proof That The Candidate Has Met All The Necessary Requirements And Is Legally Authorized To Practice As An Lvn.

  • Experience In Home Health Care: Sutter Health May Also Require Candidates To Have Prior Experience In Home Health Care. This Could Include Experience In Administering Medications, Providing Wound Care, And Assisting With Activities Of Daily Living In A Home Health Setting.

  • Knowledge Of Electronic Medical Record Systems: As Technology Becomes Increasingly Important In The Healthcare Industry, Sutter Health May Also Look For Candidates Who Have Experience With Electronic Medical Record Systems. This Would Enable The Lvn To Efficiently Document Patient Care And Communicate With Other Healthcare Professionals Within The Sutter Health Network.
