- Provide skilled nursing care: Perform a wide range of nursing tasks such as wound care, medication administration, and monitoring vital signs to ensure the safety and well-being of our home health patients.
- Conduct assessments: Conduct thorough assessments of patients' health conditions and develop individualized care plans in collaboration with the interdisciplinary team.
- Implement treatment plans: Administer prescribed treatments and therapies to patients as per the care plan and physician's orders.
- Monitor patient progress: Continuously monitor and evaluate the effectiveness of the care plan and make necessary adjustments to ensure the best possible outcomes for patients.
- Educate patients and families: Educate patients and their families about their health conditions, treatment plans, and self-care techniques to promote better understanding and involvement in the care process.
- Maintain accurate documentation: Complete all required documentation accurately and in a timely manner, including patient assessments, care plans, and progress notes.
- Communicate with the interdisciplinary team: Collaborate with the interdisciplinary team, including physicians, therapists, and social workers, to ensure coordinated and comprehensive care for patients.
- Provide emotional support: Offer emotional support and guidance to patients and their families, especially during challenging times.
- Adhere to regulations and standards: Comply with all applicable regulations and standards, including nursing practice acts, HIPAA, and agency policies and procedures.
- Attend training and meetings: Participate in ongoing training and attend meetings to stay updated on best practices and agency policies and procedures.
- Maintain professionalism: Represent the organization in a professional and compassionate manner, maintaining the highest level of integrity and ethics.

Current Lvn License: The First And Most Important Qualification For A Licensed Vocational Nurse, Home Health Job At Sutter Health Is A Current And Valid Lvn License. This Ensures That The Nurse Has Completed The Necessary Education And Training To Provide Safe And Competent Care To Patients In A Home Health Setting.
Experience In Home Health Care: Sutter Health May Require Lvns To Have Prior Experience In Home Health Care. This Can Include Experience Working With Patients In Their Homes, Providing Wound Care, Administering Medications, And Managing Chronic Conditions.
Knowledge Of Home Health Regulations: A Strong Understanding Of Home Health Regulations And Standards Is Essential For An Lvn Working In This Setting. This Includes Knowledge Of Medicare And Medicaid Guidelines, As Well As State And Federal Regulations Related To Home Health Care.
