
Inpatient Clinical Pharmacy

- Collaborate with physicians, nurses, and other healthcare professionals to ensure safe and effective use of medications for inpatient population
- Monitor and review patient medication therapy to identify and resolve any potential drug interactions or adverse effects
- Conduct medication reconciliation upon patient admission and discharge to ensure accurate and complete medication lists
- Provide drug information and education to patients, caregivers, and healthcare professionals
- Participate in interdisciplinary rounds and provide recommendations for medication therapy optimization
- Review and interpret medication orders for appropriateness and accuracy
- Prepare and dispense medications in accordance with established policies and procedures
- Participate in medication utilization evaluations and assist in developing strategies to improve medication use
- Maintain accurate and complete medication records for all patients
- Adhere to all regulatory and accreditation standards related to medication management
- Participate in quality improvement initiatives to enhance patient outcomes and safety
- Stay updated on new medications, treatment guidelines, and best practices in pharmacy practice
- Serve as a resource for other healthcare professionals and provide guidance on medication-related issues
- Maintain a safe and clean work environment and adhere to all infection control protocols
- Document all interventions, medication interactions, and adverse drug reactions in patient records.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited School Of Pharmacy.
Active And Unrestricted Pharmacist License In The State Of Employment.
Completion Of A Post-Graduate Residency Program In Inpatient Or Clinical Pharmacy.
At Least 2-3 Years Of Experience Working As A Clinical Pharmacist In An Inpatient Setting.
Knowledge And Understanding Of Pharmacotherapy Principles, Medication Management, And Drug Information Resources.
