- Provide pharmaceutical care to inpatient population: The Inpatient Clinical Pharmacist will be responsible for ensuring that patients receive safe, effective, and appropriate medication therapy during their hospital stay.
- Collaborate with healthcare team: The Inpatient Clinical Pharmacist will work closely with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive care for patients.
- Conduct medication reviews: The Inpatient Clinical Pharmacist will review patients' medication regimens to identify any potential drug interactions, duplications, or other issues that may impact patient safety or treatment outcomes.
- Dispense and compound medications: The Inpatient Clinical Pharmacist will accurately dispense and compound medications according to established protocols and guidelines.
- Educate patients and caregivers: The Inpatient Clinical Pharmacist will provide medication counseling and education to patients and their caregivers to ensure they understand their medications, potential side effects, and how to take them properly.
- Monitor medication therapy: The Inpatient Clinical Pharmacist will monitor patients' response to medication therapy and make recommendations for adjustments as needed to achieve optimal treatment outcomes.
- Ensure compliance with regulations: The Inpatient Clinical Pharmacist will adhere to all applicable laws, regulations, and professional standards in the practice of pharmacy.
- Maintain accurate records: The Inpatient Clinical Pharmacist will maintain accurate and up-to-date records of all medications dispensed and interventions made in patients' charts.
- Participate in quality improvement initiatives: The Inpatient Clinical Pharmacist will actively participate in quality improvement projects and initiatives to enhance patient care and safety.

Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution: A Pharmd Degree Is The Minimum Educational Requirement For An Inpatient Clinical Pharmacy Job At Sutter Health. This Degree Ensures That The Candidate Has Received Comprehensive Training In Pharmacotherapy, Patient Care, And Medication Management.
State Licensure: All Candidates Must Hold A Valid License To Practice Pharmacy In The State Of California. This Ensures That The Candidate Is Familiar With The State Laws And Regulations Regarding Medication Use And Dispensing.
Residency Training: Candidates With A Post-Graduate Residency Training In Clinical Pharmacy Are Preferred For Inpatient Roles At Sutter Health.
