- Provide pharmaceutical care to patients in the emergency department, utilizing extensive clinical knowledge and expertise.
- Collaborate with other healthcare professionals to ensure optimal patient outcomes through appropriate medication management.
- Conduct medication reconciliation for patients admitted to the emergency department and provide recommendations for therapy optimization.
- Monitor and assess patients' response to medication therapy and adjust treatment plans as needed.
- Ensure compliance with all state and federal regulations, as well as organizational policies and procedures.
- Participate in multidisciplinary rounds and provide drug information to healthcare team members.
- Prepare and dispense medications accurately and efficiently, checking for drug interactions and allergies.
- Stay up-to-date with developments in the field of emergency medicine and pharmacy practice, and share knowledge with colleagues.
- Maintain accurate and complete patient records, documenting all medication therapy interventions.
- Educate patients and their families on proper medication use, potential side effects, and drug interactions.
- Act as a resource for pharmacy technicians and other staff members, providing guidance and support as needed.
- Participate in quality improvement initiatives and assist with the development and implementation of new pharmacy programs.
- Maintain a safe and clean working environment, adhering to all safety and infection control protocols.
- Serve as a preceptor for pharmacy students and residents, providing guidance and teaching opportunities.
- Demonstrate a commitment to delivering the highest quality of care and promoting patient safety.
Active Pharmacist License In The State Of California
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Pharmacy Program
Board Certification In Pharmacotherapy Or Emergency Medicine
Minimum Of 5 Years Of Experience As A Clinical Pharmacist In An Acute Care Setting, With At Least 3 Years In Emergency Medicine
Strong Knowledge Of Emergency Medicine Pharmacotherapy Protocols And Ability To Provide Clinical Oversight And Leadership To Pharmacy Staff And Healthcare Team Members.
