- Review and verify medication orders for hospitalized patients to ensure accuracy, appropriateness, and safety.
- Collaborate with healthcare team members to develop and implement individualized medication therapy plans for patients.
- Provide drug information and education to healthcare professionals and patients.
- Monitor patient responses to medication therapy and make appropriate recommendations or interventions.
- Identify and resolve medication-related problems and adverse drug reactions.
- Maintain accurate and complete medication records and documentation.
- Participate in quality improvement initiatives and medication safety programs.
- Stay up-to-date with current developments in pharmacy practice and relevant healthcare policies and regulations.
- Train and mentor other pharmacy staff, including pharmacists and technicians.
- Serve as a resource for drug information and participate in multidisciplinary rounds and committees.

Doctor Of Pharmacy (Pharmd) Degree From An Accredited College Of Pharmacy.
Active And Unrestricted California Pharmacist License.
Minimum Of 5 Years Of Experience As A Clinical Pharmacist In An Inpatient Setting.
Board Certification In A Relevant Area, Such As Pharmacotherapy Or Critical Care.
Strong Clinical Knowledge And Proficiency In Medication Therapy Management, Drug Information, And Medication Reconciliation Processes.
