As the Infusion Clinical Pharmacist, you will play a vital role in collaborating with the healthcare team to develop and implement treatment plans for patients.   1. Collaborate with the healthcare team to develop and implement individualized treatment plans for oncology patients undergoing infusion therapy.
  2. Ensure the safe and effective delivery of medications to patients by accurately reviewing and verifying medication orders, monitoring for potential drug interactions, and providing appropriate dosing adjustments.
  3. Maintain accurate and organized patient records, including medication histories, treatment plans, and progress notes.
  4. Provide patient education on medication regimens, potential side effects, and drug interactions.
  5. Monitor patient response to treatment and make necessary adjustments to medication regimens as needed.
  6. Stay up-to-date with current developments in oncology pharmacy, including new medications, treatment protocols, and research findings.
  7. Work closely with other healthcare professionals, such as physicians, nurses, and pharmacists, to ensure coordinated and comprehensive care for patients.
  8. Utilize computerized systems and software to accurately and efficiently document patient information and track medication inventory.
  9. Adhere to all legal and ethical standards, as well as company policies and procedures.
  10. Act as a resource for other healthcare professionals regarding medication-related inquiries and concerns.
  11. Participate in quality improvement initiatives to improve patient outcomes and enhance the delivery of care.
  12. Maintain a clean and organized workspace, ensuring compliance with all safety and infection control protocols.
  13. Provide excellent customer service to patients and their families, addressing any concerns or questions they may have.
  14. Collaborate with insurance companies and other healthcare organizations to ensure timely and appropriate payment for services.
  15. Attend trainings, conferences, and other educational opportunities to continuously improve knowledge and skills.

Job Qualifications
  • Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution

  • Board Certification In Oncology Pharmacy (Bcop)

  • Current State Licensure As A Pharmacist In The State Of California

  • Minimum Of 2-3 Years Of Experience In A Hospital Or Infusion Center Setting, With A Focus On Oncology

  • Strong Knowledge Of Chemotherapy Regimens, Drug Interactions, And Adverse Effects Related To Cancer Treatment.
