
Hospice Aide

- Provide compassionate and high-quality care to patients receiving hospice services.
- Assist patients with personal care, including bathing, grooming, dressing, and toileting.
- Administer medication and perform basic medical procedures, as directed by a licensed nurse.
- Monitor and document patients' vital signs and report any changes or concerns to the appropriate medical personnel.
- Assist with mobility and transfers, using proper body mechanics and safety techniques.
- Provide emotional support and companionship to patients and their families during their end-of-life journey.
- Maintain a clean and safe environment for patients, following infection control and safety protocols.
- Collaborate with the interdisciplinary team to develop and implement a care plan that meets the individual needs of each patient.
- Communicate effectively with patients, families, and team members to ensure the best possible care.
- Attend and participate in ongoing education and training programs to maintain and improve knowledge and skills.
- Adhere to all HIPAA regulations and maintain patient confidentiality at all times.
- Demonstrate a commitment to the mission and values of Sutter Health and the hospice program.
- Maintain accurate and timely documentation of all patient care provided.
- Respond to emergency situations calmly and efficiently, following established protocols.
- Continuously evaluate and reassess patient needs and report any concerns or changes to the appropriate team members.
Certified Nursing Assistant (Cna) Or Home Health Aide (Hha) Certification
Experience Working In Hospice Care Or With Terminally Ill Patients
Compassionate And Empathetic Attitude Towards Patients And Their Families
Ability To Provide Personal Care And Assistance With Daily Living Activities
Strong Communication And Interpersonal Skills To Effectively Work With A Multidisciplinary Team And Communicate With Patients And Their Families.
