- Provide diabetes education and support to patients and their families to help them effectively manage their condition.
- Develop and implement education plans tailored to each patient's individual needs and level of understanding.
- Conduct assessments to evaluate patient's knowledge and understanding of diabetes management.
- Collaborate with other healthcare professionals to develop and implement comprehensive care plans for patients with diabetes.
- Stay updated on the latest research, treatments, and technologies related to diabetes management.
- Serve as a resource for patients and their families regarding diabetes management, including medication, nutrition, physical activity, and lifestyle changes.
- Maintain accurate and thorough documentation of patient interactions and progress.
- Conduct group education sessions for patients with diabetes and their families.
- Provide emotional support and encouragement to patients as they navigate the challenges of managing their condition.
- Participate in community outreach programs to promote diabetes education and prevention.
- Adhere to all ethical and professional standards of practice.
- Continuously evaluate and improve diabetes education programs to ensure the highest quality of care for patients.
- Comply with all regulatory requirements and maintain patient confidentiality at all times.
- Attend training and professional development opportunities to enhance knowledge and skills in diabetes education.
- Act as a positive and professional representative of Sutter Health at all times.
Bachelor's Degree In Nursing, Nutrition, Or A Related Field.
Certification As A Diabetes Educator (Cde) Through The National Certification Board For Diabetes Educators (Ncbde).
Minimum Of 3 Years Experience Working With Patients With Diabetes In A Clinical Or Educational Setting.
Strong Knowledge Of Diabetes Management, Including Medication Management, Nutrition, And Exercise.
Excellent Communication And Teaching Skills, With The Ability To Educate And Support Patients Of Diverse Backgrounds And Cultures.
