  • Develop and maintain strong relationships with community members, organizations, and partners to promote and implement community health initiatives.

  • Collaborate with a multidisciplinary team to develop and implement effective strategies and programs to address community health issues.

  • Document and maintain accurate records of community interactions, program outcomes, and progress towards goals.
  • Participate in community events and health fairs to raise awareness and promote healthy living.
  • Stay up-to-date on current public health trends and research to inform the development of evidence-based interventions.
  • Serve as a liaison between the community and healthcare providers to facilitate access to healthcare services and resources.
  • Uphold the values and mission of Sutter Health and promote a positive image of the organization in the community.

Job Qualifications
  • Bachelor's Degree In Public Health, Community Health, Or Related Field.

  • Minimum Of 2 Years Experience Working As A Community Health Worker Or In A Similar Role.

  • Knowledge And Understanding Of Community Health Issues And Resources.

  • Strong Communication And Interpersonal Skills, With The Ability To Build Relationships And Engage With Diverse Populations.

  • Experience With Program Development, Implementation, And Evaluation In A Healthcare Setting.
