- Review and analyze medical records to ensure accuracy and completeness of clinical documentation.
- Collaborate with healthcare providers to identify and clarify incomplete or unclear documentation.
- Utilize coding and classification systems to accurately assign diagnoses and procedures.
- Stay up-to-date on industry standards and regulations related to clinical documentation.
- Communicate with coding and billing teams to ensure accurate coding and billing based on clinical documentation.
- Educate healthcare providers on the importance of accurate and complete clinical documentation.
- Identify and report any patterns or trends in documentation discrepancies to improve overall quality and compliance.
- Conduct audits and provide feedback to healthcare providers to improve documentation practices.
- Maintain confidentiality and adhere to HIPAA regulations when handling sensitive patient information.
- Participate in departmental and organizational meetings and trainings as required.
- Utilize electronic health record systems and other technology to efficiently manage and track documentation tasks.
- Collaborate with other members of the healthcare team to provide comprehensive and coordinated care for patients.
- Assist in developing and implementing policies and procedures related to clinical documentation.
- Maintain accurate and organized records of all documentation activities.
- Continuously seek opportunities to improve documentation processes and contribute to the overall improvement of patient care.
Bachelor's Degree In Healthcare-Related Field: A Clinical Documentation Specialist At Sutter Health Should Have A Minimum Of A Bachelor's Degree In A Healthcare-Related Field Such As Nursing, Health Information Management, Or Medical Coding.
Certified Clinical Documentation Specialist (Ccds) Credential: Sutter Health May Require Candidates For This Role To Hold A Ccds Certification, Which Demonstrates Knowledge And Expertise In Clinical Documentation Improvement.
Experience In Healthcare Documentation And Coding: Candidates Should Have At Least 2-3 Years Of Experience Working In Healthcare Documentation And Coding, Preferably In A Hospital Or Clinical Setting.
Knowledge Of Icd-10 And Cpt Coding: A Strong Understanding Of Icd-10 And Cpt Coding Systems Is Essential For A Clinical Documentation Specialist, As They Will Be Responsible For Ensuring Accurate And Compliant Coding For All Patient Records.
Strong Analytical And Problem-Solving Skills: This Role Requires The Ability To Analyze And Interpret Medical Records And Identify Any Discrepancies Or Gaps In Documentation. Candidates Should Possess Excellent Critical Thinking And Problem-Solving Skills To Effectively Improve Clinical Documentation Processes.
