- Provide compassionate and high-quality care to hospice patients in their homes.
- Work as a part of a hospice team to ensure coordinated and personalized care.
- Assist patients with daily living activities, including bathing, grooming, and feeding.
- Monitor and record vital signs and report any changes to the hospice nurse.
- Administer medications and treatments as directed by the hospice nurse.
- Provide emotional support and companionship to patients and their families.
- Communicate effectively with patients, families, and other healthcare professionals.
- Maintain accurate and timely documentation of patient care.
- Follow all hospice policies and procedures to ensure a safe and comfortable environment for patients.
- Attend regular team meetings and participate in care planning discussions.
- Continuously assess and report any concerns or changes in patient's condition to the hospice nurse.
- Participate in ongoing education and training to maintain certification and enhance skills.
- Maintain a professional and empathetic demeanor at all times.
- Adhere to all HIPAA regulations and maintain patient confidentiality.
- Collaborate with other healthcare providers to ensure the highest quality of care for patients.
- Advocate for patients and their families to ensure their needs are being met.
- Provide support and guidance to family members and caregivers.
- Maintain and respect cultural, religious, and personal beliefs and values of patients and their families.
- Promote a positive and comforting atmosphere for patients and their families.

Completion Of A State-Approved Home Health Aide Training Program: A Certified Home Hospice Aide At Sutter Health Must Have Completed A State-Approved Training Program And Received A Certificate As Proof Of Their Knowledge And Skills In Providing Home Health Care.
Current Certification And Registration: The Candidate Must Possess A Valid Certification And Registration As A Certified Home Health Aide In The State Where They Will Be Providing Care. This Ensures That They Are Up-To-Date With The Latest Regulations And Protocols In Home Health Care.
Prior Experience In Hospice Care: Sutter Health Seeks Candidates With Prior Experience In Hospice Care, Preferably In A Home Setting. This Experience Will Demonstrate The Candidate's Ability To Provide Compassionate And Specialized Care To Patients In The Comfort Of Their Own Homes.
Excellent Communication And Interpersonal Skills: As A Certified Home Hospice Aide, The Candidate Will Be Working Closely With Patients, Their Families, And Interdisciplinary Care Teams.
