- Provide compassionate and holistic care to patients in their homes.
- Work closely with the interdisciplinary team to develop and implement personalized care plans for patients.
- Assist patients with activities of daily living, including personal hygiene, grooming, and mobility.
- Administer medication and assist with pain management as directed by the supervising nurse.
- Monitor and report changes in patients' physical, mental, and emotional status to the supervising nurse.
- Provide emotional support and companionship to patients and their families during their end-of-life journey.
- Maintain a safe and clean environment for patients, following all infection control and safety protocols.
- Document all care provided accurately and in a timely manner.
- Participate in ongoing training and education to maintain HHA certification and stay updated on hospice care practices.
- Adhere to all Sutter Health policies and procedures, as well as state and federal regulations.
- Maintain strict confidentiality and respect for patients' privacy and rights.
- Communicate effectively and professionally with patients, families, and team members.
- Show empathy, patience, and a positive attitude towards patients and their loved ones.
- Help patients achieve a comfortable and dignified end-of-life experience.
- Collaborate with community resources to ensure patients receive the best possible care and support.
Completion Of A State-Approved Home Health Aide Training Program: A Certified Home Health Aide (Hha) Must Have Completed A State-Approved Training Program That Includes A Minimum Of 75 Hours Of Training, Including Classroom Instruction And Hands-On Clinical Experience.
Valid Certification And License: A Hha Must Possess A Valid Certification As A Home Health Aide And Be Licensed By The State In Which They Intend To Practice. This Ensures That The Individual Has Met All The Necessary Qualifications And Standards To Provide Quality Care To Patients.
Knowledge Of Hospice Care: A Hha Working At Sutter Health Should Have A Deep Understanding Of Hospice Care And The Unique Needs Of Hospice Patients. This Includes Knowledge Of Pain Management, End-Of-Life Care, And Emotional Support For Both Patients And Their Families.
Experience In Home Health Or Hospice Settings: Prior Experience In A Home Health Or Hospice Setting Is Often Preferred By Employers. This Demonstrates That The Hha Has Practical Experience Providing Care To Patients In Their Own Homes And Is Familiar With The Challenges And Responsibilities Of This Type Of Work.
