- Conduct initial and ongoing assessments of patients seeking psychiatric services.
- Collaborate with a multidisciplinary team to develop individualized care plans for patients.
- Coordinate and facilitate referrals to appropriate mental health services and resources.
- Provide support and advocacy for patients navigating the healthcare system.
- Monitor patient progress and adjust care plans as needed.
- Maintain accurate and up-to-date documentation of patient interactions and care plans.
- Educate patients and their families on mental health resources and coping strategies.
- Collaborate with community organizations to provide additional support and resources for patients.
- Follow ethical and legal standards when handling confidential patient information.
- Stay up-to-date on best practices and trends in mental health and case management.
- Communicate effectively with patients, colleagues, and other healthcare professionals.
- Adhere to Sutter Health's mission, values, and policies.
- Participate in team meetings and case conferences to discuss patient care and progress.
- Advocate for patients' rights and facilitate access to necessary services.
- Maintain a high level of compassion, empathy, and professionalism in all interactions with patients.
Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or Related Field.
Minimum Of 2 Years Experience In Psychiatric Intake, Assessment, And Referral.
Knowledge Of Mental Health Diagnoses, Treatment Options, And Resources.
Strong Communication And Collaboration Skills With Multi-Disciplinary Teams And Community Organizations.
Current Licensure Or Certification In Mental Health Field (E.g. Lcsw, Lmft, Lpc, Etc.).
