  2. Utilize strong clinical knowledge and expertise to assess patients' needs and develop individualized care plans.
  3. Advocate for patients and collaborate with other healthcare professionals to ensure the best possible outcomes.
  4. Monitor and evaluate patients' progress, adjusting care plans as needed.
  5. Educate patients and their families on their conditions, treatment plans, and available resources.
  6. Communicate effectively with patients, families, and healthcare team members to ensure continuity of care.
  7. Maintain accurate and thorough documentation of patient care.
  8. Adhere to all relevant laws, regulations, and ethical standards in healthcare.
  9. Participate in quality improvement initiatives and contribute to the development of best practices.
  10. Stay current with industry trends, advancements, and best practices in case management.
  11. Collaborate with insurance providers and other third-party payers to ensure appropriate coverage and reimbursement for patients.
  12. Maintain a high level of compassion, empathy, and professionalism when interacting with patients and their families.
  13. Continuously strive to improve patient outcomes and satisfaction.

Job Qualifications
  • Current Registered Nurse (Rn) License In The State Of Employment.

  • Bachelor's Degree In Nursing (Bsn) Or Related Field.

  • Minimum Of 2 Years Of Experience As A Case Manager In A Healthcare Setting.

  • Experience With Utilization Review, Discharge Planning, And Care Coordination.

  • Knowledge Of Medicare, Medicaid, And Managed Care Regulations And Guidelines.
