This role plays a crucial part in assisting our case managers in providing high-quality care to our patients with mental health needs.   1. Support Case Managers: The Case Management Assistant II will work closely with case managers to support their daily tasks and responsibilities. This may include organizing and maintaining patient files, scheduling appointments, and assisting with communication between patients, families, and healthcare providers.
  2. Provide Administrative Support: This role will be responsible for completing administrative tasks such as data entry, scheduling meetings, and preparing reports. The Case Management Assistant II will also assist with managing insurance authorizations and coordinating with other departments within the healthcare system.
  3. Participate in Patient Care: The Case Management Assistant II will play an active role in the care of patients with mental health needs. This may include attending meetings with the treatment team, participating in patient rounds, and providing emotional support to patients and their families.
  4. Maintain Confidentiality: As a member of the healthcare team, the Case Management Assistant II will be responsible for maintaining strict confidentiality of patient information and following all HIPAA guidelines.
  5. Collaborate with Other Departments: This role will require collaboration with other departments within the healthcare system, including social workers, therapists, and physicians. The Case Management Assistant II will ensure timely and effective communication to provide comprehensive care to patients.
  6. Uphold Quality Standards: The Case Management Assistant II will be responsible for upholding the quality standards set by Sutter Health.   8. Attend Trainings and Meetings: This role may require attending trainings and meetings to stay updated on new policies, procedures, and best practices in the field of mental health.
  9. Ensure Patient Satisfaction: The Case Management Assistant II will play a crucial role in ensuring patient satisfaction by providing compassionate and empathetic support to patients and their families.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Or A Related Field.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Multidisciplinary Team.

  • 2+ Years Of Experience In Case Management Or A Related Field, Preferably In A Healthcare Setting.

  • Strong Understanding Of Mental Health And Psychiatric Disorders.

  • Experience With Electronic Medical Records And Case Management Software.
