  1. Assist the case management team in coordinating patient care by scheduling appointments, arranging transportation, and communicating with healthcare providers.
  2. Maintain accurate and organized patient records, including medical history, treatment plans, and progress reports.
  3. Collaborate with other departments and healthcare professionals to ensure a seamless and efficient care experience for patients.
  4. Provide excellent customer service to patients and their families by addressing any questions or concerns and ensuring their needs are met.
  5. Participate in team meetings and contribute ideas for improving processes and enhancing patient care.
  6. Follow all HIPAA regulations and maintain confidentiality of patient information.
  7. Assist with insurance verification and authorization processes.
  8. Help patients navigate the healthcare system by providing them with necessary resources and information.
  9. Monitor and track patient progress and assist in identifying any barriers to care.
  10. Continuously update job knowledge by participating in educational opportunities and staying current with industry trends.
  11. Maintain a positive and professional attitude while working in a fast-paced and dynamic environment.
  12. Adhere to all company policies, procedures, and protocols.
  13. Perform any other duties as assigned by the case management team.

Job Qualifications
  • Education: A Minimum Of A High School Diploma Or Equivalent Is Required For This Position. A Bachelor's Degree In A Related Field Such As Social Work Or Healthcare Administration Is Preferred.

  • Experience: Previous Experience Working In A Healthcare Setting, Specifically In Case Management Or Care Coordination, Is Highly Desired. Familiarity With Medical Terminology And Electronic Medical Records Is Also Preferred.

  • Organizational Skills: The Role Of A Case Management Assistant Involves Managing Multiple Tasks And Responsibilities Simultaneously. Strong Organizational Skills Are Essential To Ensure Timely And Accurate Completion Of Tasks And Documentation.

  • Team Player: The Ideal Candidate For This Role Should Be A Team Player Who Can Collaborate Effectively With Other Members Of The Healthcare Team. This Includes Working Closely With Case Managers, Nurses, Social Workers, And Other Healthcare Professionals To Coordinate Care For Patients.
