
Care Manager, Registered Nurse

- Coordinate and manage the care of patients to ensure the highest quality of care and support throughout their healthcare journey.
- Collaborate with healthcare team members to develop and implement individualized care plans for patients.

- Provide education and support to patients and their families regarding their medical conditions, treatment plans, and resources.

- Utilize evidence-based practices and clinical guidelines to deliver safe and effective care.
- Conduct regular evaluations of patient outcomes and work with the team to identify areas for improvement.
- Maintain accurate and thorough documentation of patient care and progress.
- Ensure compliance with all regulatory and organizational standards, policies, and procedures.
- Serve as a resource for patients, families, and other healthcare professionals.
- Participate in interdisciplinary team meetings and contribute to the development of policies and procedures.
- Stay up-to-date with current healthcare trends and advancements in care management.

Bachelor's Degree In Nursing From An Accredited Institution
Active And Unrestricted Registered Nurse (Rn) License In The State Of California
Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Case Management Or Care Coordination
Strong Knowledge Of Healthcare Regulations And Reimbursement Systems, Including Medicare And Medicaid
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Patients, Families, And Interdisciplinary Healthcare Teams.
