- Provide clinical pharmacy services in an ambulatory setting, including medication therapy management, drug information, and patient education.
- Collaborate with physicians, nurses, and other healthcare providers to optimize patient outcomes and ensure safe and effective medication use.
- Conduct patient assessments and develop individualized care plans to address medication-related issues and promote patient adherence.
- Utilize electronic health records and other technology to efficiently and accurately document patient care activities.
- Stay up-to-date on the latest developments in pharmacy practice, including new medications, treatments, and guidelines.
- Serve as a drug information resource for healthcare providers, patients, and caregivers.

Doctor Of Pharmacy Degree (Pharmd) From An Accredited Institution
Active Pharmacist License In The State Of California
Minimum Of 5 Years Of Ambulatory Clinical Pharmacy Experience
Board Certification In Ambulatory Care (Bcacp) Or Other Relevant Specialty Area
Strong Knowledge Of Current Pharmacy Practices, Regulations, And Guidelines In Ambulatory Care Settings
