- Conduct comprehensive medication therapy management for patients with chronic conditions, utilizing evidence-based guidelines and protocols.
- Collaborate with other healthcare professionals to develop and implement individualized treatment plans for patients.
- Provide patient education on medication use, potential side effects, and potential drug interactions.
- Monitor and evaluate patient response to therapy and make appropriate recommendations for adjustments.
- Participate in medication reconciliation processes to ensure accurate and safe transition of care.
- Stay up-to-date on industry developments, new drugs, and treatment options to provide the most current and effective care to patients.
- Maintain accurate and complete patient records, including medication histories and treatment plans.
- Adhere to all state and federal regulations and laws regarding dispensing and administering medications.
- Act as a resource and mentor for other pharmacists and healthcare professionals.
- Participate in quality improvement initiatives and help identify opportunities for enhancing patient care.
- Collaborate with insurance providers to obtain prior authorizations and ensure cost-effective and appropriate medication regimens for patients.
- Maintain a safe and clean work environment and adhere to all safety protocols.
- Act as an advocate for patients and their medication needs.
- Continuously strive to improve patient outcomes and satisfaction.
- Attend and actively participate in team meetings, trainings, and other professional development opportunities.
Doctor Of Pharmacy (Pharmd) Degree From An Accredited Institution
Current And Active State Pharmacist License
Board Certification In Ambulatory Care Pharmacy Or Related Specialty (E.g. Geriatrics, Diabetes, Etc.)
Minimum Of 3-5 Years Of Experience As A Practicing Pharmacist In An Ambulatory Multi Specialty Clinic Setting
Advanced Knowledge Of Pharmaceuticals, Drug Interactions, And Medication Management In A Primary Care Setting.
