- Provide person-centered care and support to individuals with developmental disabilities.
- Develop and implement individualized habilitation plans to meet the unique needs and goals of each individual.
- Monitor and document progress towards goals, and make necessary adjustments to plans as needed.
- Collaborate with a multidisciplinary team to ensure the holistic care and well-being of individuals.
- Participate in training and development opportunities to enhance skills and knowledge in the field of developmental disabilities.
- Maintain a safe and supportive environment for individuals to promote their independence and well-being.
- Communicate effectively with individuals, their families, and other team members to ensure continuity of care.
- Advocate for the rights and needs of individuals with developmental disabilities.
- Adhere to all state and federal regulations, policies, and procedures related to the care and support of individuals with developmental disabilities.
- Act as a role model and mentor for individuals, promoting their personal growth and independence.
- Continuously assess and evaluate the effectiveness of habilitation plans and make recommendations for improvements.
- Maintain accurate and timely documentation of all services provided.
- Attend and participate in staff meetings, trainings, and other team activities.
- Represent the organization in a professional and positive manner at all times.
- Perform any other duties as assigned by the supervisor or management.
Bachelor's Degree In A Related Field Such As Social Work, Psychology, Or Special Education
Experience Working With Individuals With Disabilities And Knowledge Of Habilitation Techniques
Knowledge Of State And Federal Regulations Related To Disabilities Services
Strong Communication And Interpersonal Skills To Effectively Work With Clients, Families, And Other Professionals
Ability To Develop And Implement Individualized Habilitation Plans And Evaluate Progress Towards Goals.
