- Provide direct support and assistance to individuals with disabilities in their daily activities.
- Develop and implement individualized plans to meet the specific needs and goals of each individual.
- Conduct assessments and evaluations to identify areas of improvement and develop appropriate strategies.
- Collaborate with a team of professionals, including therapists, social workers, and medical personnel, to ensure comprehensive care for individuals.
- Monitor and document individual progress, behaviors, and incidents in accordance with agency guidelines.
- Foster a positive and inclusive environment that promotes independence, self-determination, and socialization.
- Assist with personal care tasks, including grooming, hygiene, and medication administration.
- Advocate for the rights and needs of individuals with disabilities and ensure their voices are heard.
- Provide training and guidance to individuals on a variety of life skills, such as cooking, budgeting, and community integration.
- Maintain a safe and clean environment for individuals, following all health and safety protocols.
- Attend trainings and workshops to stay up-to-date on best practices and new developments in the field of disability services.
- Uphold the mission and values of the agency and adhere to all policies and procedures.
- Engage in professional development opportunities and seek out opportunities for growth and advancement within the agency.
Minimum Of 2 Years Of Experience Working With Individuals With Developmental Disabilities.
Bachelor's Degree In A Related Field Such As Social Work, Psychology, Or Special Education.
Knowledge Of State And Federal Regulations And Policies Related To Habilitation Services.
Strong Communication And Interpersonal Skills To Effectively Work With Individuals With Disabilities, Their Families, And Other Service Providers.
Ability To Develop And Implement Individualized Habilitation Plans And Document Progress.
