
Habilitation Specialist 1

- Develop individualized plans for individuals with developmental disabilities to enhance their quality of life and promote their independence.
- Conduct assessments to determine the specific needs and goals of each individual.
- Collaborate with other team members, such as case managers and therapists, to ensure comprehensive and coordinated care.
- Implement and monitor progress of individualized plans, making adjustments as needed.
- Provide direct support and assistance to individuals with daily activities such as personal care, socialization, and recreational activities.
- Advocate for individuals' rights and needs, ensuring they are treated with dignity and respect.
- Maintain accurate and up-to-date records of services provided and progress made.
- Educate and train individuals in life skills to increase their independence and self-sufficiency.
- Communicate and collaborate with families and caregivers to ensure a holistic approach to care.
- Stay up-to-date on best practices and guidelines for working with individuals with developmental disabilities.
- Adhere to all state and federal regulations and policies related to the care of individuals with developmental disabilities.
- Attend meetings, trainings, and conferences to enhance knowledge and skills.
- Participate in the development and improvement of programs and services for individuals with developmental disabilities.
- Foster a supportive and inclusive environment for individuals, promoting their overall well-being.
- Continuously evaluate and assess the effectiveness of services provided and make recommendations for improvement.
Bachelor's Degree In A Related Field Such As Social Work, Psychology, Or Special Education.
At Least 2 Years Of Experience Working With Individuals With Developmental Disabilities Or Mental Health Disorders.
Knowledge Of State And Federal Laws And Regulations Related To Habilitation Services.
Strong Communication And Interpersonal Skills, With The Ability To Work Collaboratively With A Diverse Team.
Certification As A Habilitation Specialist Or Willingness To Obtain Certification Within The First Year Of Employment.
